The following groups are eligible to apply for funding as the Lead Agency (or co-lead) and participate in the Request for Proposal (RFP) process for Integrated Youth Services (IYS) initiatives: Non-Profit Organizations, Charitable Organizations, Band/Tribal Councils, Indigenous Communities, Municipalities, and Post Secondary Institutions.

Family Care Team Partner

Family Care Teams are an innovative approach to re-imagine and re-design the health system in Newfoundland and Labrador. Family Care Teams aim to improve access and continuity of primary health care (PHC) for individuals and families in their community. They represent a significant shift from solo based community practice and program-based models of service delivery to an inter-disciplinary team-based model. Family Care Teams offer seamless access to multiple health care professionals that focus on meeting the health and social needs of individuals and families. Family Care Teams are an essential component of the IYS model.

3. Your Proposed IYS Hub

• Please describe the vision for your proposed Youth Hub. Please ensure that the vision is aligned with the Ten Principles of IYS, found in the Guidelines (Appendix B)
• In your response please include:
• How the proposed IYS Hub will help your community improve services and outcomes for children, youth, and young adults. (aged 12-29)
• How the proposed hub will reach a broad catchment area (demographically and regionally). This may include a combination of pop-up and outreach work and/or virtual services.
• How your vision aligns with the IYS Network vision, concept and service components detailed in the "Purpose and Design of IYS" section, found in the Request for Proposals Guideline document.

Please list committed, planned, and potential partnerships (such as Family Care Team/NLHS, community agencies, Indigenous Governments or communities, Government partners), as well as details of the services for youth and families that will be provided by your proposed IYS Hub.

Clearly identify which element(s) of the IYS model will be provided by which organization(s), including estimated Full-Time Equivalents (FTEs) and in-kind and cash contributions.

4. Youth and Family Engagement

Meaningful engagement of youth and families is a critical element of the IYS model and all IYS hubs.

Please describe:

• Your organization’s and/or community partners’ experiences working collaboratively with youth and their families.
• How your proposed IYS Hub will engage and work with youth and families to ensure meaningful participation in service development and delivery on an ongoing basis.

5. Diversity, Inclusion, Cultural Safety, Health, and Social Equity

Please tell us about your previous experience engaging historically marginalized/vulnerable youth, racialized, Indigenous or 2SLGBTQIA+ identified youth, and/or youth with disabilities.

Please describe how your IYS Hub will work to integrate principles of access, equity, and inclusion and work to ensure access to culturally appropriate services that meet the diverse needs of your community.

6. Active Participation in a Provincial IYS Network

Key components of Newfoundland and Labrador’s IYS strategy include:

• A common evaluation framework, standardized measures, data management, data submission processes and continuous quality improvement; and
• Hub participation in the development and implementation of a common branding strategy for Newfoundland and Labrador’s IYS Initiative.
• CFY data and evaluation strategy incorporating the use of a client-based data management system (EMHWare) to gather, measure and analyse client data including intake, intervention, impact, and discharge data. This data set works in conjunction with other knowledge gathering methods such as surveys, focus groups, research, and external data.
• Please describe how your proposed IYS Hub will contribute to these activities:

• What challenges do you anticipate in implementing a data/ evaluation plan that aligns with a provincial network?

Funding will be provided across two new IYS Hubs to support the transition to offering integrated services, and to enhance services to achieve alignment with the IYS vision, values, critical service elements and desired outcomes.

Include the anticipated or projected funding that will be required for IYS Hub operations, coordination, and service delivery over three fiscal years:

Include the following budget breakdowns in your proposal:

• Year 1:
• Estimated cost of renovations/retrofits, furniture, and other up-front costs, to be completed and/or implemented by Spring 2025
• Estimated operational investment, starting September 2024. Operational investments include cost of materials, rent, utilities, salaries, and benefits, etc.
• Proposals may not exceed $1,390,650 in capital investment, and $340,306 in operational investment (year 1).
• Year 2 and beyond:
• Annual operating budget, including rent, utilities, materials, salaries, and benefits, etc.
• Annual operating budgets (year 2 and beyond) may not exceed $580,612 (annually). This does not include any NLHS contribution.

8. Community Support and Endorsement

Community Letters of Support

Letters that illustrate support for your proposal is required with your submission. One endorsement letter must come from Newfoundland and Labrador Health Services (from applicant’s regional zone), and you are encouraged to collect and submit additional letters from community leaders, key partners, and/or young people to further support your proposal. These letters should demonstrate a commitment to working together and making changes in the system, which is crucial to establishing an IYS Hub. Please be sure to include these letters, statements, and messages with your proposal submission.
